Usage Question
We are excited about your interest in our tools, and you are very welcome to use them in your practice subject to the Terms of Use below. Our tools are intended to be publicly and freely available for clinic/patient use. If you want to use them as is, there is no need to ask for permission – this use is covered under the creative commons license. If you would like to make modifications to our tools, we require that you contact us first for permission. No existing content on the decision aid may be removed. If a signature line is requested for a patient to sign in order to acknowledge that the patient has read through the decision aid, please create a cover sheet for this purpose. It is not acceptable to add a signature line to the decision aid itself.
Changes that have been deemed unacceptable at any level and will violate our terms of use:
- Making a profit from charging persons for use of the decision aids.
- Biasing the decision aid to promote one brand of device/medication or promote one decision over another (ie: getting a device or not getting a device)
- Including any device manufacturing brand information of any type.
Other changes may be possible, but require a discussion with the contact below. One modification that has been deemed acceptable in the past and would likely be agreed to again is adding a hospital or site logo to the decision aid. This still requires an agreement. Furthermore, by requesting modification, you agree to document reasons for any changes and you agree to allow the Colorado Program for Patient Centered Decisions to study the implementation and use of any changed versions of the decision aids.
