Target audience: LVAD Coordinators, Nurses, Social Workers, Advance Practice Providers, and Physicians who work with patients with LVADs and their caregivers.
Program Overview: ENABLE (Educate, Nurture, Advise Before Life Ends) is an evidence-based early palliative care program. In this self-paced course, you will learn how to provide ENABLE to caregivers of patient with left ventricular assist device (LVADs).
This course consists of an introduction and 4 modules (approximately 4 hours) following the Charting Your Course (CYC) booklet. Modules will consist of videos, readings, learning activities, and other resources. Modules will: 1) provide background information, 2) give content for you to then share with caregivers, 3) share tips and best practices for communicating with caregivers, and (4) allow you to practice skills.
Course Objectives:
Introduction
- Describe the ENABLE intervention.
- Discuss why caregivers are an important component of care for patients with LVADs.
Module 1: How to Handle Problems
- Describe COPE attitude components.
- Describe 7-steps to problem-solving technique.
Module 2: Taking Care of You
- Describe the effects of self-care.
- Review resources for caregivers of patients with heart failure.
Module 3: Being a Partner in Symptom Management
- Describe common symptoms of patients with heart failure.
- Identify support resources for caregivers of patients with an LVAD.
Module 4: Talking about What Matters Most & Making Choices
- Explain the role of core values in health care decision making.
- Describe the use of decision aids in clinical practice.
- Identify the role of the caregiver in advance care planning and surrogate decision making.
